[Intravascular ultrasound-guided percutaneous treatment in a case of spontaneous coronary artery dissection].

چکیده
A 33-year-old woman presented an anterior myocardial infarction treated with fibrinolytics. On coronary angiography performed a few days later, a dissection of the left anterior descending artery (LAD) was observed, with no lesions in other vessels. The patient was stable and showed no symptoms; hence revascularization treatment was not performed at that time. One week later the patient returned to the hospital with angina and transient ST segment reelevation from V2 to V6. A new coronary angiography was undertaken. The results were similar to those seen previously, except for an image of a double lumen with an intimal flap from the proximal segment of the LAD to the distal portion, with contrast retention and grade II TIMI flow (Figure 1). Coronary stent implantation was chosen to treat the dissection; nevertheless, during the procedure it was difficult to determine whether the guidewire was advancing through the true or false lumen. Spontaneous Coronary Artery Dissection Manifested during Ergonovine Test and Treated with Intravascular Ultrasound Guided Stenting: A Case Report
Spontaneous coronary artery dissection (SCAD) is an uncommon cause of acute myocardial ischemia, which frequently presents as sudden death. The pathophysiology and treatment of SCAD have not been fully determined. Herein, a case of SCAD, manifesting as variant angina, which rapidly progressed during an ergonovine test, in which 3 drug-eluting stents were deployed using intravascular an ultrasou...
متن کاملSpontaneous Coronary Artery Dissection/Intramural Haematoma in Young Women with ST-Elevation Myocardial Infarction: “It Is Not Always a Plaque Rupture Event”
Spontaneous coronary artery dissection (SCAD) is an unusual, but increasingly recognized, cause of ST-elevation myocardial infarction (STEMI), especially among younger patients without conventional risk factors for coronary artery disease (CAD). Although dissection of the coronary intima or media is a hallmark finding, hematoma formation within the vessel wall is often present. It remains uncle...
متن کاملPercutaneous Coronary Intervention in Spontaneous Coronary Artery Dissection: Role of Intravascular Ultrasound
Spontaneous coronary artery dissection (SCAD) is a rare, life-threatening condition that usually manifests as an acute myocardial infarction. Diagnosing SCAD with conventional coronary angiogram can be challenging, particularly if the true lumen is severely narrowed. Our case highlights the challenges in performing successful percutaneous coronary intervention (PCI) in patients with SCAD. Intra...
متن کاملSpontaneous coronary artery dissection by intravascular ultrasound in a patient with myocardial infarction
Spontaneous coronary artery dissection is a rare cause of acute coronary syndrome and sudden cardiac death. The incidence, causes, pathogenesis, and treatment have not been defined clearly, but spontaneous coronary artery dissection should be considered in young patients without major cardiovascular risk factors or in patients in the peripartum period who present with acute coronary syndrome. T...
متن کاملThe role of intravascular ultrasound guidance in the treatment of intramural hematoma probably caused by spontaneous coronary artery dissection in a young woman with acute anterior myocardial infarction.
Spontaneous coronary artery dissection (SCAD) is known to be a rare but fatal cause of acute coronary syndromes. It is more frequent in young women, particularly in the peripartum period. Intravascular ultrasound (IVUS) has an important role in the diagnosis and management of SCAD. Intramural hematoma that occurs between adventitial and media layer of the vessel wall may occlude the true lumen....
